＜追加成績証明書をご希望の皆さま＞

いつもJSAF-IELTSで受験いただきありがとうございます。

IELTSを受験後、追加成績証明書を依頼される場合の注意事項についてお知らせいたします。

例年7月以降、追加成績証明書の発行依頼を多数いただく時期となりますが、追加成績証明書はこちらのウェブサイトより（https://jsaf-ieltsjapan.com/requestforms/）申請フォームを受領してから、5営業日以内に、申請順にご対応いたします。提出期限のある方は、なるべく余裕を持ったIELTS受験および申請を強くお勧めいたします。

なお、追加成績証明書の申請フォームは、スコア結果が開示されてから申請していただくようお願いいたします（スコアが開示されてない状態で申請フォームをお送り頂いた場合は受理されません）。

また、IELTS受験申込時にRecognising Organisationsにて出願先を登録されている場合は、結果開示後、登録している出願先へスコアを発送いたします（万が一、登録した出願先への送付を取りやめたい場合は、成績開示後24時間以内にお申し出いただきますようお願いいたします。）

＜To all who may request an additional Test Report Form＞

Thank you for taking the JSAF-IELTS test.

We would like to inform you of a few important points when requesting an additional Test Report Form after taking the IELTS test.

From July onward, we usually receive many requests for additional Test Report Form.

We will process all requests for additional Test Report Form in the request order, within 5 business days after we receive the request form from this website: https://jsaf-ieltsjapan.com/request-forms/. If you have a deadline to submit your Test Report Form, it is highly recommended that you take the IELTS test and submit your application as far in advance as possible.

Please note that your request would be submitted only after the score results to be released before submitting your request for an additional Test Report Form. If you send us your request form before the release of your score results, it will not be accepted.

If you have registered Recognising Organizations when booking your test, we will send your Test Report Form to the Receiving Institutions after the score results are released. In case you do not wish to have your score results sent to the Receiving Institutions, please notify us within 24 hours after the score results are released.
